Youth Ministries Participation Form
Providing for the safety of youth is our most important ministry. Parents and guardians are asked to provide the information and permission needed at the beginning of the school year to help ensure the care and safety of their 6th through 12th graders participating in youth ministries.
Families with youth need to complete and return Youth Emergency Contact and Family Info forms for each 6th-12th grader in their family. This form is kept confidential and on file through July 2018. No youth may participate in an overnight or off-site event without current forms on file.
2017/2018 Emergency Contact and Information Form Don't forget to submit your form WITH A COPY OF YOUR INSURANCE CARD (FRONT/BACK). Only one copy of your card is needed per family. You can submit your form as an attachment and email to Amy Roberts. Or deliver by hand to Christ Church Guest Services Desk.
Working With Youth
Christ Church makes every effort to create a safe, protected environment for our children and youth. One way we do this is through the Safe Sanctuaries ministry. All adult paid staff and all adults who lead or serve with youth must undergo a background check and screening process and complete a Safe Sanctuaries application and training. We make sure there are multiple trusted adults around our children and youth at all times.
