Children's Ministries Family Information Form

The sharing of information essential to your child’s safety has been streamlined into one form, allowing you to provide information and give permission once for your family, as well as provide detailed information regarding any medical needs. Every participating child, birth through age 18, will need a family Children's Ministries Family Information Form on file. All ministry areas will make use of the information you provide through this form.

Once completed, please return your forms ALONG WITH A COPY OF YOUR INSURANCE CARD. You can scan and email your forms to Kathryn Vecchio, or bring them to church in a sealed envelope and deliver them to the Guest Services Desk.

Child Check-in System

The safety and security of our children is very important at Christ Church. We use an electronic Check-In system to provide name tags for children and claim checks for their parents. When families arrive, they go straight to one of the four Check-In stations and print tags for each child. Check-In stations can be found

  • In the Gathering Hall, outside the Sanctuary
  • Outside the Nursery
  • In the preschool hallway, outside Room 107
  • In the elementary hallway, outside Room 006

All adults picking up children are required to turn in their claim check to a Children's Ministries leader. If you do not have a claim check, you may be asked to show ID.

To enroll in electronic Check-In, please complete and return a Contact & Emergency Care Form for your family, available below.

Fall Harvest Snack Collection

To ensure children get a nice healthy snack each week, each family is asked to bring ONE of the following during October:

Children's List:
Quaker Chewy Chocolate Chip Granola Bars
Mott's Medleys Assorted Fruit Snacks
Rice Krispies Treats
Mini Milla wafers

Your choice of snack your teen prefers.

Working with Children

Christ Church makes every effort to create a safe, protected environment for our children and youth. One way we do this is through the Safe Sanctuaries ministry. All adult paid staff and all adults who lead children undergo a background check and screening process and complete a Safe Sanctuaries application and training. We make sure there are multiple trusted adults around our children at all times.
